The Michael Bartlett Award for Community Impact
Launched in 2019 as a legacy to Michael Bartlett’s dedication to MLSE Foundation, this award will recognize and celebrate an MLSE Foundation community partner that demonstrates incredible leadership and commitment to both measurable and meaningful impact in the community. 
 
Eligibility
· Individual is from an MLSE Foundation partner of more than one year
· Individual demonstrates above and beyond leadership in their community
· Community impact highlights work with children and youth
· Nomination must be written by someone other than the individual nominated
 
Nomination
Must Include:
· Personal information page form
· Two-page letter maximum Nomination Statement (see breakdown below)
· Two letters of support from community members
(can be attached to the nomination not included in two-page maximum)
 
Nomination Statement
· What nominee has done/is doing to deserve a nomination, including summary of activities, when they occurred and for how long
· How the activities go above and beyond the duties for which the individual is paid
· Speak to the sustainability of the impact created due to their work in community
· How the activities have created significant impact for children and youth 
· How the activities have created significant impact in the community 
 
 
How We Give
1. Open call to all community partners to submit nominations
2. Submissions will be reviewed by internal MLSE Foundation team
3. MLSE Foundation will deliberate, and selected recipient will be approved by the MLSE Foundation Board of Directors
 
Award
· $5,000 donation from MLSE Foundation to the organization of the recipient’s choice
· Presented with a Michael Bartlett Award for Community Impact
· Hosted for a dinner at an MLSE restaurant by MLSE Foundation representative followed by an in-game experience and two tickets for one of MLSE’s team games
